Disciple Maker Index (DMI)

The Disciple Maker Index Survey opens on October 17th. 
Your response is most helpful to MMR Parish!

  • (Note that the link will be open from October 17th through November 16th)

• Paper copies of the survey may be picked up in the Church Narthex.
• Paper copies are also available at the Parish Office during business hours.
  • (Monday-Thursday 8:30 am-8:00 pm, and Friday 8:30 am-4:00 pm)
• Instructions on returning completed paper surveys:
  • Paper surveys must be returned to the Parish Office by November 19, 2020.
